Minimally invasive dentistry (MID) marks a transformative shift in oral healthcare, centered on early diagnosis, preventive strategies, and conservative interventions that prioritize preservation of natural tooth structure and long-term patient wellbeing. This integrative review combines evidence from paediatric, general, and esthetic dentistry to highlight the evolution, techniques, and materials underpinning MID. In paediatric care, approaches such as silver diamine fluoride, resin infiltration, atraumatic restorative treatment, and bioactive materials have demonstrated efficacy in managing caries with minimal trauma, especially in resource-limited or anxiety-prone populations. Diagnostic innovations including laser fluorescence, electrical conductivity, and chemo mechanical caries removal support early lesion detection and non-invasive management. In aesthetic rehabilitation, enamel-limited preparations and feldspathic ceramic laminates offer long-term functional and visual success, as evidenced by 12+ years of clinical follow-up. Across all domains, bioactive agents and technologies like 3D printing and artificial intelligence enhance precision, remineralization, and patient-centered care. While challenges such as cost, clinician training, and material sensitivity persist, minimally invasive dentistry continues to redefine clinical benchmarks merging biological preservation with esthetic precision and expanding access across public health settings.
